Custom HTML form - how to add menu to user dashboard

can someone please let me know how to add menu item to user dashboard, which leads to custom HTML form ?

Can you please help me :

- add the new link to the members menu to the left. Click on this link opens the new page with web-form
- create page with the web form (contact entry)


I want to make possible to members to add contacts into the database.

I am using Admidio as auth for PHP contacts web application
