Found the answer.

The database has to be set up beforehand using the web site control panel. There is no mention of this requirment in the installation sequence documentation and no helpful message to say what is wrong.

After filling in the first page with the database name the installation script moves on to the next page (Define Organisation) and sticks there with nothing happening.

What should happen? Any idea why it isn't happening?
